i Deadlines, fees, and what happens if you are late

When it is due
During the licensee's birth month in each even-numbered year. 18VAC85-20-230(A) requires renewal "biennially during his birth month," and the fee regulation 18VAC85-20-22 sets the medicine/osteopathic medicine renewal fee as due in the licensee's birth month in each even-numbered year.
Fee
$337 (active MD/DO biennial renewal; inactive license renewal is $168) per 18VAC85-20-22
Late fee
$115 additional fee for processing a late renewal application within one renewal cycle (active license); $55 per cycle for late renewal of an inactive license (18VAC85-20-22)
If you miss the deadline
No stated grace period for practice. A license not renewed in the birth month can be late-renewed within one renewal cycle by paying the $337 renewal fee plus the $115 late fee. Once lapsed two successive years or more, late renewal is no longer available: the physician must file a reinstatement application, pay the $497 reinstatement fee, and attest to CME equal to the requirement for the years lapsed (capped at 4 years' worth) per 18VAC85-20-240. If out of active practice more than four years, the Board may additionally require passage of SPEX or COMVEX-USA. Reinstatement after suspension/revocation or after a denied petition is $2,000.
Processing time
Effectively immediate for online renewal: the Board is paperless and states the renewed status displays on License Lookup as soon as the fee is validated (Board FAQ). No paper license is mailed.

2 Virginia CME Requirements

30 hours per 2-year cycle.

Tracking your CME across multiple states is one of the most time-consuming parts of credential management. CredentialDOMD automatically monitors your CME progress and alerts you when you're falling behind, so you never miss a requirement.

3 Common Pitfalls to Avoid

These are the mistakes Virginia physicians most commonly make during renewal:

Track your Virginia license, CME, and DEA in one place

The reminder email above covers this renewal. The app goes further: it watches your VA renewal window, counts your CME hours against the requirement, and tracks your DEA registration alongside your license. Built by a practicing neurosurgeon who uses it daily. Free during beta.

4 Frequently Asked Questions

When is my Virginia medical license due for renewal?
Biennially during your birth month in each even-numbered year (18VAC85-20-230 and 18VAC85-20-22). A renewal notice is mailed roughly 45-60 days before expiration.
How much does renewal cost?
$337 for an active MD or DO license; $168 to renew an inactive license (18VAC85-20-22). Online payment accepts Visa, MasterCard, American Express, or Discover.
What happens if I miss my birth-month deadline?
Within one renewal cycle you can still renew by paying the $337 fee plus a $115 late fee. Once the license has been lapsed two successive years or more, you must apply for reinstatement instead: $497 fee, reinstatement application, and attestation of back CME up to four years' worth (18VAC85-20-240).
Do I get a paper license after renewing?
No. The Board is paperless; your renewed status displays on the DHP License Lookup as soon as your fee is validated (Board FAQ).
Do I submit CME certificates with my renewal?
No. You attest to completing continued competency hours at renewal. The Board randomly selects licensees for post-renewal audit and gives a time frame to produce documentation, so keep your certificates.
Where do I renew online?
At the DHP Online Licensing portal, https://www.license.dhp.virginia.gov/license/Login.aspx. First-time users register at the New Users link using the PIN and license number from the mailed renewal notice.
