When it is due

MD: "Physician licenses expire on January 31 of even years." DO: "Osteopathic Physician and Surgeon license renewals are due May 31st of even years." Controlled substance licenses renew at the same time as the main license. (Next renewal for both: 2028, since the 2026 dates have passed.)

Fee

$193 (MD or DO, 2-year renewal). Add $78 to renew a Utah controlled substance license (total $271). Verified on the FY 7/1/2025-6/30/2026 DOPL fee schedule ("Physician and Surgeon ... Renewal Fee 193.00"; "Osteopathic Physician and Surgeon 200.00 / 193.00"; "Controlled Substance 100.00 / 78.00") and stated as "$193 renewal fee" on the DO renewal page. Fees are non-refundable and "subject to change each July 1."

Late fee

No separate late-renewal window; a lapsed license is a reinstatement. Within 30 days after expiration: renewal fee PLUS $20 for EACH license being reinstated. After 30 days and within 2 years: renewal fee PLUS $50 for EACH license (from the DOPL Renewal/Reinstatement form v.20231107; matches the fee schedule's "Late Renewal Fee $20.00 / Reinstatement Fee $50.00"). The physician license and the controlled substance license are separate licenses, so the per-license fee applies to each.

If you miss the deadline

No grace period: under Utah Code 58-1-308(2)(b) "A license that is not renewed prior to the expiration date shown on the license automatically expires." Reinstatement is available within 2 years of nonrenewal (renewal fee + reinstatement fee + proof of renewal qualifications); a license "reinstated no later than 120 days after it expires shall be retroactively reinstated to the date it expired" (58-1-308(5)(c)). After 2 years you must qualify for a new license (58-1-308(6)(a)), with a 6-month exception for those practicing under federal employment or another state license after reestablishing Utah domicile. Reinstatement cannot be done online; DOPL requires the manual Renewal/Reinstatement form. Practicing after expiration is unlawful conduct and a criminal offense (Utah Code 58-1-501(1)(a), 58-1-502), and DOPL may discipline for time worked unlicensed even after reinstatement.

3 Common Pitfalls to Avoid

These are the mistakes Utah physicians most commonly make during renewal:

MD and DO deadlines are different: MD licenses expire January 31 of even years, DO licenses May 31 of even years. A dual-boarded or converting physician can miss one of the two.

The controlled substance license is a separate license renewed at the same time ($78 on top of the $193). If you lapse, the per-license reinstatement penalty ($20 within 30 days, $50 after) is charged for EACH license, so an MD+CS lapse costs double penalties.

There is no grace period. The license automatically expires on the expiration date (Utah Code 58-1-308(2)(b)), and continuing to practice is a criminal offense (58-1-501/58-1-502); DOPL can discipline for unlicensed practice even after you reinstate.

Reinstatement and re-application cannot be done through the online portal; DOPL requires the paper Renewal/Reinstatement form mailed with fees. Only on-time renewal is online.

Renewal notices go only to the email address of record (DOPL emails the notice 60-90 days before expiration), and Utah Code 58-1-301.7 requires you to report any address or email change within 10 business days. An out-of-date email means no renewal notice.

DOPL audits on a random-sample basis.

Can I keep practicing while my reinstatement is processing? ▾

No. Practicing after expiration is unlawful conduct and a criminal offense under Utah Code 58-1-501(1)(a) and 58-1-502, and DOPL may take disciplinary action for any period worked without a license even after it is renewed or reinstated. The only protection is the retroactive effect of a reinstatement completed within 120 days.
