No grace period: ORC 4731.281 states failure to renew "shall operate automatically to suspend the holder's license to practice." Lapsed under 2 years: reinstatement is mandatory ("the board shall reinstate") on submission of a renewal application and the $405 reinstatement fee. Lapsed 2 years or more: restoration is discretionary ("the board may restore") and requires a restoration application, $505 fee, FBI and Ohio BCI fingerprint background checks sent directly from the vendor, an AMA (MD) or AOA (DO) physician profile report, and completion of the 50-hour CME requirement in the 24 months preceding the restoration application. Board allows 10 business days for initial review of a restoration application.
