The completed renewal application, fee, and supporting documents must be filed on or before the expiration of the license (RSA 310:8, III). OPLC must send renewal notice at least 2 months before expiration (RSA 310:8, IV), and the OPLC renewal page says licensees get an email renewal notification 90 days before expiration.

If you miss the deadline
No grace period and no late-renewal window. Under RSA 310:8, III, if the completed renewal, fee, and documents are not filed by the expiration date the license expires and "the holders of an expired license are not authorized to practice until the licenses have been reinstated." Reinstatement fee for a physician is $413.00 ($378.00 for a Compact Physician) per the OPLC fee page. Expired licenses lapse one year from expiration; after that the holder cannot renew and must reinstate by submitting an application and meeting eligibility requirements in statute and board rules.

Do I have to do anything besides the renewal application and fee? ▾
Yes. Physicians renewing an active-status license must complete the NH Physician Licensure Survey or the opt-out form through DHHS, required by RSA 126-A:5, XVIII-a (administrative and inactive licenses are exempt). Your CME (100 hours per 2-year cycle, at least 40 Category 1, plus 3 hours in pain management/addiction disorders for NH-DEA holders) is now tracked through CE Broker, which OPLC made mandatory for the Board of Medicine as of February 3, 2025.
