Late fee
Nebraska has no late-renewal fee as such; a missed deadline means the license is expired and must be reinstated. Reinstatement adds a $35 reinstatement fee on top of the $121 renewal fee ($156 total per the fee chart on the 01/2026 reinstatement application, prorated to $65.25 if reinstating April through August of an even-numbered year). Separately, practicing on an expired license incurs an administrative penalty of $10 per day up to $1,000.
If you miss the deadline
No grace period. Per the DHHS renewal page: if you missed your renewal deadline, your license has expired and you cannot practice until it is reissued or reinstated; practicing after expiration incurs a $10 per day administrative penalty up to $1,000. Reinstatement requires the combined renewal + reinstatement fee, 50 Category 1 CE hours already completed within the 24 months before applying, and proof of ONE of the following within the 3 years preceding application: 1 year of active practice (any US state, DC, territory, or Canada), 1 year of approved GME, Board-approved CE, a Board-approved refresher course, or the special purposes examination. The Board of Medicine and Surgery does not routinely accept CE or the special purposes exam alone as evidence of continued competency absent recent practice; under Neb. Rev. Stat. 38-2026.01, a physician who has not actively practiced for the 2 years preceding application may be offered only a reentry license, and declining the reentry license results in denial of the application. A reinstated license expires on the same October 1 of the next even-numbered year.

No. DHHS no longer mails paper renewed license cards. After your renewal processes (up to about a week if no additional documentation is needed), print your wallet card from the license lookup at dhhs.ne.gov/lookup.
Do DOs renew through a separate osteopathic board? ▾
No. Nebraska licenses osteopathic physicians under the same Medicine and Surgery Practice Act, and MDs and DOs use the same renewal notice, the same reinstatement application, the same fees, and the same October 1 even-year cycle through the DHHS Licensure Unit.
