When it is due

Fixed date: all physician licenses expire January 31 of every year. Renewal information is mailed and/or emailed on or before December 1. A renewal is timely if the fee is received before expiration; forms postmarked February 1 or after are delinquent, except that when January 31 falls on a weekend or legal holiday, a postmark on the next business day is not delinquent (20 CSR 2150-2.001(17)).

Fee

$147 (Permanent Physician Renewal Fee, MD and DO alike, 1-year registration) per 20 CSR 2150-2.080(1)(A)4. All fees are nonrefundable.

Late fee

$25 Late Renewal Fee (Delinquent Fee) per 20 CSR 2150-2.080(1)(B)4; the board may waive it for extenuating circumstances such as illness (section 334.090, RSMo), but failure to receive a renewal notice is expressly not an extenuating circumstance (20 CSR 2150-2.001(8))

If you miss the deadline

No practice grace period: once expired you cannot practice in Missouri until renewal is granted. Late renewal remains possible through MOPRO with the $25 delinquent fee. If registration lapses more than 6 months, the renewal application is denied unless accompanied by all required fees plus a completed renewal application made under oath with full disclosure of disciplinary actions and current physical and mental fitness (20 CSR 2150-2.050); the fee schedule lists a $75 Reinstatement Fee. If the license is not renewed for two consecutive renewal periods it becomes VOID (section 334.080, RSMo); a void license may not be renewed or reactivated (20 CSR 2150-2.001(18)). A license revoked, suspended, or inactive more than 2 years can require 25 hrs CME per lapsed year plus a competency exam (SPEX, specialty board exam, or CPEP/PACE assessment) for reinstatement (20 CSR 2150-2.150); inactive more than 5 years may trigger reexamination (20 CSR 2150-2.153).
