When does my Michigan medical license expire?

Three years from the date it was issued, on the anniversary of the issue date (R 338.7002). The exact date is printed on your license and shown in your MiPLUS account; LARA notes expiration dates vary by licensee. LARA sends a renewal notice by mail or email, but failure to receive it does not excuse a late renewal (MCL 333.16201(1)).

How much does renewal cost and how do I pay?

$314.40 for the 3-year MD or DO renewal per the LARA fee schedule, plus $248.10 if you also renew a controlled substance license. Payment must be by Visa, Mastercard, American Express, or Discover credit or debit card in MiPLUS. The MD guide page lists $314.00, but the fee schedule and current DO guide both say $314.40.

How early can I renew?

No sooner than 90 days before your expiration date, online at www.michigan.gov/miplus (MiPLUS login at aca-prod.accela.com/MILARA). You must register a MiPLUS account, using your name exactly as it appears on your license, before you can renew. Renewals generally process within a few days.

What happens if I miss my expiration date?

You have a 60-day grace period during which you may still renew by paying the renewal and late renewal fees and completing your CME, and you may legally continue to practice during those 60 days (MCL 333.16201(2)). After 60 days the license is null and void: you cannot practice, and you must apply for relicensure ($395.00) through MiPLUS with proof of 150 CME hours. A lapse over 3 years adds exam (SPEX for MDs, COMVEX for DOs) or re-entry program requirements unless you are actively licensed in good standing in another state.

Do I have to submit my CME certificates when I renew?

No. You attest at renewal that you completed the required CME (150 hours per 3-year cycle: minimum 75 Category 1 for MDs, 60 Category 1 for DOs; 3 hours pain and symptom management including 1 hour controlled substance prescribing; 1 hour medical ethics; plus 1 hour per year implicit bias training, one-time human trafficking training, and opioid awareness training for controlled substance licensees). Keep documentation at least 4 years; the Board randomly audits after each renewal period. BPL partners with CE Broker for optional tracking.

Do MDs and DOs renew differently?

Both renew through the same MiPLUS system, on the same 3-year issue-date cycle, at the same $314.40 fee, under separate boards. The main difference is CME composition: MDs need at least 75 of 150 hours in Category 1; DOs need at least 60 of 150 hours in Category 1 (per R 338.143, at least 40 of those AOA or MOA approved).

Late fee

Amount not published for physicians. MCL 333.16201(2) requires payment of "renewal and late renewal fees" to renew within 60 days after expiration, but neither the current MD/DO licensing guides nor the LARA fee schedule states the dollar amount for MD/DO (LARA's FAQs for some other BPL professions have cited $20.00, which could not be verified for physicians from a currently published page).

If you miss the deadline

60-day grace period: a license not renewed by expiration may be renewed within 60 days after the expiration date on application, payment of renewal and late renewal fees, and completion of CME; the physician MAY continue to practice during those 60 days (MCL 333.16201(2)). After 60 days the license is null and void and practice is prohibited (MCL 333.16201(3)). Relicensure via MiPLUS "Modification": $395.00 (MD or DO relicensure fee per the 2026 licensing guides) plus proof of 150 hours CME in the prior 3 years. Lapsed more than 3 but less than 5 years: also need an active license in good standing in another state OR one of SPEX (COMVEX for DOs), a board-approved postgraduate program, or an accredited physician re-entry program. Lapsed 5+ years: active out-of-state license OR BOTH the exam and a training/re-entry program. Relicensure applicants whose license expired within the last 3 years skip the criminal background check.

Processing time

Renewals are generally completed within a few days (LARA MiPLUS user assistance page); new licenses average 4-6 weeks.

This state runs separate MD and DO requirements. MD: 150 hours per 3-year cycle: at least 75 hours in Category 1, 1 hour of ethics, 3 hours of pain management, 1 hour of implicit bias, required topics: Human Trafficking, Opioid Awareness.
