Last day of the licensee's birth month, in even or odd years matching the licensee's birth year: physicians born in an even-numbered year expire on the last day of their birth month in an even-numbered year, those born in an odd-numbered year in an odd-numbered year (MD board FAQ). The DO board also sets the renewal date by date of birth per its application instructions; the DO statute (32 MRS 2581-1) leaves the exact schedule to board rule.

$500 (MD active license, 2-year renewal). DO: $525 biennial renewal; $0 for DOs age 70+ (CME still required). Initial licenses expiring within 6 months of issuance renew at a prorated $150 (MD).
Late fee
$100 (MD): renewal accepted up to 90 days after expiration with the additional $100 late fee. DO board publishes no separate late fee; reinstatement after lapse is $550 ($525 renewal plus $25 administrative fee).
If you miss the deadline
MD: 90-day late-renewal window after expiration with a $100 late fee; after 90 days the license lapses. Lapsed under 5 years: reinstatement application, $600 total ($50 reinstatement + $500 renewal + $50 late), plus evidence of 12 months of active practice or passing a competency exam and 40 CME credits in the prior 24 months; reinstatement from withdrawn status is $550. Lapsed over 5 years: full new application. How much does renewal cost? ▾
$500 for an MD renewal with the Board of Licensure in Medicine. DOs pay $525 to the Board of Osteopathic Licensure, and DOs age 70 or older on January 1 of the renewal year pay $0 (CME is still required).
Where do I renew online? ▾
MDs renew at the state licensing portal, board_number=376 (licensing.web.maine.gov); DOs use the same portal system with board_number=383. You need your license number and access code, and pay the fee online.
What happens if I miss my renewal deadline? ▾
MDs may still renew within 90 days of expiration by paying an additional $100 late fee. After 90 days the license lapses: lapsed under 5 years requires a reinstatement application ($600 total from lapsed status) plus proof of 12 months of active practice or a competency exam and 40 CME credits in the prior 24 months; lapsed over 5 years requires a completely new application.

Yes, periodically. The MD board's online portal requires renewing licensees to complete an open-book examination on board rules every 4 years, i.e. at every other renewal.
What CME do I need to renew? ▾
MDs: 40 AMA Category 1 hours per 2-year cycle, including 3 hours of opioid prescribing CME; current ABMS certification with MOC can satisfy the 40 hours. DOs: 100 hours per 2-year cycle with at least 40 hours of osteopathic medical education, plus the 3-hour opioid requirement for prescribers (32 MRS 2600-C).
