When it is due
Fixed date for all licensees: renewal is due by March 1 of each year. Reminder notices are mailed to all active licensees in late December. Renewals paid after March 1 but before April 1 require a penalty fee; failure to renew by April 1 makes the license automatically inactive.
Fee
$150 (annual registration or renewal of regular MD/DO license, online). Paper renewal adds $10. Limited license renewal is $65.
Late fee
$50 penalty if paid March 1 to April 1; $100 penalty after April 1 (at which point the license has also automatically become inactive). Per KBML Board Fees PDF and 201 KAR 9:041.
If you miss the deadline
Late window March 1 to April 1 with a $50 penalty on top of the $150 fee. After April 1 the license automatically becomes inactive and further practice is considered the unauthorized practice of medicine or osteopathy. Reactivation cannot be done online: the physician must contact the Board at (502) 764-2607 or [email protected] for a re-registration form; the fee for re-registration of an inactive license is $250. The online application is for new applicants only.

2 Kentucky CME Requirements

3 Common Pitfalls to Avoid

These are the mistakes Kentucky physicians most commonly make during renewal:

Track your Kentucky license, CME, and DEA in one place

The reminder email above covers this renewal. The app goes further: it watches your KY renewal window, counts your CME hours against the requirement, and tracks your DEA registration alongside your license. Built by a practicing neurosurgeon who uses it daily. Free during beta.

4 Frequently Asked Questions

When is my Kentucky medical or osteopathic license renewal due?
By March 1 every year. Reminder notices are mailed to all physicians holding an active license in late December, and the online renewal is completed through the Board's eBusiness portal. Renewals paid after March 1 but before April 1 require an extra $50 penalty; after April 1 the license automatically becomes inactive.
How much does renewal cost?
$150 for annual registration or renewal of a regular license. Renewing by paper application adds $10. The late penalty is $50 between March 1 and April 1, and $100 after April 1. Limited license renewal is $65. All amounts are from the Board Fees PDF, which mirrors 201 KAR 9:041.
How do I log in to renew online?
Go to the KBML eBusiness portal (services.kbml.ky.gov/ebusiness). Your username is your Kentucky license number and your password is the last four digits of your SSN. Do not create a new account; renewing licensees already have one. Payment is by credit card or ACH debit, and the payment screen opens as a pop-up, so allow pop-ups.
What happens if I miss the April 1 deadline?
The license automatically becomes inactive and continued practice in Kentucky is considered the unauthorized practice of medicine or osteopathy. You cannot reactivate online: contact the Board at (502) 764-2607 or [email protected] for a re-registration form. The re-registration fee for an inactive license is $250.
I hold an IMLC compact license. Do I renew with KBML?
No. The Board states that physicians holding a license issued through the Interstate Medical Licensure Compact must renew on the IMLCC website (imlcc.org/renew), not through the KBML portal.
How do I prove I am renewed while waiting for the record to update?
Print the temporary wallet card and receipt at the completion of online payment by choosing the Finish Transaction With KBML option; the Board no longer mails wallet cards and the card cannot be reproduced once the receipt screen closes. The renewed license appears on the Board's online verification site in approximately 48 hours.
