When it is due

License expires on the first day of the licensee's birth month; renewal issues a two-year license that again expires on the first day of the birth month (Iowa Admin. Code r. 481-652.12(3)). Online renewal opens no more than 60 days before the renewal date.

Fee

$450 if renewed online, $550 if renewed by paper application, per biennial period (prorated if the current license was issued for less than 24 months); $0 for physicians on active duty in the U.S. armed forces, reserves, or national guard during the renewal period. A $7 online transaction fee applies to online purchases (Iowa Admin. Code r. 481-651.4(1)c, 481-651.1).

Late fee

$100 per calendar month after the license expiration date, up to a maximum of $200 (Iowa Admin. Code r. 481-651.4(1)d).

If you miss the deadline

A physician who misses the expiration date may still renew during a two-month penalty window ($100/month, max $200). If not renewed by the first day of the third month after expiration, the license becomes inactive and invalid, and practice is prohibited (r. 481-652.12(4), 481-652.13(2)). Reactivation within one year of becoming inactive costs $550 and requires a renewal application plus full CME documentation (r. 481-652.14(1)). Reactivation after one year or more costs $500 plus a $45 fingerprint/background-check fee and requires a full reactivation application, 40 hours of Category 1 CME in the previous two years, and, if out of active clinical practice for 3+ years, a board-approved competency evaluation, SPEX/COMVEX, retraining, or reentry program (r. 481-652.14(2)). A license not renewed or reactivated within five years after expiration is deemed relinquished and can never be reinstated; the physician must apply for a brand-new license (r. 481-652.16).
