When it is due
Licensee's birthdate. As of April 1, 2026, DOPL began transitioning the Board of Medicine to biennial licenses that expire on the licensee's birthday, phased in by odd/even birth year so renewals spread across two cycles. Before this transition, DOPL licenses renewed on a June cycle (1-year or 2-year licenses in the month of June). Renewal can be completed up to 60 days before the birthday expiration. Physicians mid-transition should confirm their individual expiration date in the eDOPL portal.

$160 per year for both Physician and Surgeon (MD) and Osteopathic Physician and Surgeon (DO), effective January 1, 2025 (reduced from $200). If you miss the deadline
No published grace period or separate late fee. An unrenewed license lapses. A license lapsed less than 5 years may be reinstated by submitting a renewal application with ALL back renewal fees, a $35 reinstatement fee, and proof of required CME. A license lapsed more than 5 years requires a completely new licensure application.
