When it is due
All Physician and Pro Bono Physician licenses expire on April 30 of odd-numbered years, regardless of when the license was issued (next expiration: April 30, 2027). Renewal opens in the DPO Online Services portal approximately 4 to 6 weeks before the expiration date (the Applications page says renewals open about 4-5 weeks prior; the DPO FAQ says the renewal link appears when the license is within 6 weeks of expiration).
If you miss the deadline
Colorado law (CRS 12-20-202(1)(e), loaded from the official 2024 CRS Title 12 printout) gives a 60-day grace period after expiration: you may renew without disciplinary sanction for practicing on the expired license during those 60 days, but you must satisfy all renewal requirements and pay a delinquency fee. If you do not renew within the 60-day grace period, the license is treated as Expired and you are ineligible to practice until reinstated (CRS 12-20-202(2)). Reinstatement (per the board's Medical Licensing Guide and Reinstatement application) requires the reinstatement application and fee, a current certified NPDB self-query report, an FSMB Disciplinary Action Report sent to DORA, out-of-state license verifications, a chronological practice history for the last year, liability insurance attestation, and, if you have not actively practiced for the last year, a continued-competency narrative under Board Rule 1.8. If the license has been expired more than 2 years, CRS 12-20-202(2)(c)(II) additionally requires a satisfactory demonstration of competency (active out-of-state license in good standing, restricted practice, remedial coursework, CME, examination, or other measures the Board accepts). Active military personnel and non-dishonorably-discharged veterans get mandatory reinstatement if they meet the statutory requirements.

When can I renew, and how? ▾

The board states renewals open approximately 4-5 weeks before the expiration date, and the DPO FAQ says a 'Complete License Renewal' link appears in your account once the license is within 6 weeks of expiration. If the link is not visible, your license is not yet in its renewal period.
What happens if I miss the April 30 deadline? ▾
You have a 60-day statutory grace period to renew without disciplinary sanction for practicing on the expired license, but you must pay a delinquency fee on top of the renewal (CRS 12-20-202(1)(e)). After 60 days the license is Expired: you are ineligible to practice and must file a reinstatement application with an NPDB certified report, an FSMB Disciplinary Action Report, practice history, and insurance attestation. Expired more than 2 years adds a competency demonstration requirement.
How much is the renewal fee? ▾
The board does not publish the physician renewal or delinquency fee amount on its website; the exact fee displays in the DPO Online Services portal when you renew and on the application forms. All fees are subject to review and change on July 1 each year and are nonrefundable (dpo.colorado.gov/Medical/DRLicenseRequirements, Fees/Deadlines tab).
I am a DO. Do I renew through a different board? ▾
No. Colorado has a single Colorado Medical Board that licenses and renews both MDs and DOs through the same DPO Online Services portal on the same April 30 odd-year schedule. There is no separate osteopathic board in Colorado.
Do I need malpractice insurance to renew or reinstate? ▾
Colorado law requires licensed physicians to maintain professional liability coverage of at least $1,000,000 per incident and $3,000,000 annual aggregate, or to qualify for an exemption under Board Rule 1.14 (for example, residing and practicing outside Colorado).
